Record Keeping
During hypnotherapy sessions, notes will be taken to document progress. As required by law, these records will be securely stored for seven (7) years from the date of your last session. All records are kept in a secure location to maintain confidentiality and prevent unauthorized access.
Confidentiality & Limitations
Your privacy is of the utmost importance to us. However, there are specific circumstances where we may be legally or ethically required to disclose information:
If we believe you are at risk of harming yourself or another person.
If we suspect that a child may be in danger.
If you disclose involvement in a criminal offense.
If we receive a court subpoena requiring the release of your records.
If you provide written consent for us to share your information with a colleague (e.g., for a referral).
Access to Your Records
You have the right to request access to your session notes under the following conditions:
Your request must be made in writing and must state the purpose of the request.
A minimum of 48 hours' notice is required to retrieve and provide access to your records.
